Ascendant New York is a free-standing boutique detox, inpatient rehabilitation and outpatient facility located in Midtown Manhattan. Ascendant New York is licensed by the State of New York’s Office of Addiction Services and Supports and is looking for a full-time Case Manager-CASAC. Monda-Friday, 11:00 AM-7:30 PM.
Job Description:
As a Case Manager- CASAC at Ascendant NY, you'll play a pivotal role in our Intensive Outpatient Program (IOP). Your responsibilities will include conducting intake, screening, and assessments for new clients, and once admitted, you'll facilitate a smooth transition by connecting clients to their assigned therapist. Additionally, you'll offer personalized peer support, mentorship, and advocacy to guide individuals through their recovery journey here at Ascendant NY. As a Case Manager- CASAC, you will be an integral part of the discharge planning process and next level of care, working closely with the clinical team. Your approach will be rooted in person-centered recovery principles, incorporating trauma-informed care, medication-assisted treatment, and peer support methodologies.
Job Responsibilities:
· Learn, uphold, and model all policies and procedures pertaining to the Ascendant NY program;
· Working with the Ascendant OP Admissions team, schedule prospective clients for intake and assess for history of substance use, intoxication, suicidal/homicidal risk, reason for treatment, appropriateness for IOP or OP level of care, including the completion of the LOCADTR and completing biopsychosocial assessments and other assessment screening tools;
· Monitor the collection of the UDS and the toxicology results;
· Working closely with Clinicians, overseeing coordination of care for clients, to bridge the gap between clinical care and case management;
· Collaborate with Program Director and Clinician’s regarding critical cases and interventions;
· Work closely with clinicians to facilitate effective discharge planning and next level of care for clients;
· Working with the Program Director, guide weekly outreach to educate the community and host monthly events for the Ascendant and recovery community;
· De-escalate and address any clinical issues that may arise in waiting area;
· Provide support crisis intervention and support when needed;
· Model healthy coping skills and educate clients on various pathways to recovery;
· Attend weekly group supervision and individual supervision;
· Ability to participate in community outreach including working closely with Ascendant’s inpatient detox and rehab facility;
· Other appropriate duties as may be assigned by the Program Director.
Job Requirements:
· Associate’s degree required, Bachelor’s degree preferred;
· Valid NYS Credentialed Alcohol and Substance Abuse Counselor (CASAC) certification;
· Ability to motivate clients in a courteous, empathic and professional manner;
· Excellent listening and relationship-building skills to establish strong working alliances with clients and staff;
· Superior communication skills to convey information clearly, accurately, and completely;
· Ability to de-escalate and maintain professional standards when handling difficult outreach and case management challenges;
· Must have experience with integrated substance abuse assessment, treatment and motivational interviewing techniques;
· Ability to thrive in a collaborative multi-disciplinary team setting and attend supervision, seminars, and meetings as needed;
· Computer savvy with experience with MS Office, web-based EHR systems, and entering timely documentation;

Pay: $62,000.00 - $70,000.00 per year
