POSITION SUMMARY:
Responsible for providing wrap around services to high risk youth by providing case management to youth and their families.
DUTIES AND RESPONSIBILITIES:

Meet with clients at bedside at Los Angeles County Hospital until discharge
-
Meet at the client's home and office as needed to identify the client’s needs and resources available to the individual and family.

Assist in coordinating and leading groups of youth to pro-social activities as needed.

Provide one on one individual coaching/counseling to guide the client through successes and challenges.
-
Establish and maintain good working relationships with CBO’s, schools, probation and/or and stakeholder working with clients.

Attends and participates in all mandatory meetings.

Enters data into participant databases; assists in the writing of periodic program reports.

Responsible for assisting participants and their families in making referrals to partner organizations.

Maintain accurate and up to date files in the database and hard copy.
-
Respond and follow up to referrals made by other agencies or individuals.

Maintain accurate and detailed case notes regarding client’s progress or any relevant information to the case planning.

Attend weekly case management staff meetings to discuss client’s progress. Including team meetings.

Follow up with clients was discharge to ensure services are being met and provided

Evaluating clients’ progress periodically and making adjustment as need to improve outcomes
-
Maintains client files.

Gathers and verifies documents to verify program criteria are met for services.

Provides a long term plan of action for each client which includes direct services and referral services, including, neighborhood-based case management, relocation and transition, educational support, referrals to housing, mental health, substance abuse treatment, and tattoo removal etc.

Advocates for the client when needed.

Works with parents/guardians/siblings and provides services to the family, if needed.

Coordinates with program coordinators as needed.

Maintains confidentiality and comprehensive confidential case files.

Monitors agencies to which clients are referred to assure quality service.

Works collaboratively with other case managers.

Coordinates and collates monthly program reports.

REQUIREMENTS

Fully vaccinated for COVID-19 as outlined in the contract requirements

Comply with Hospital Background/Clearance as outlined in the contract requirements
Any combination of experience and education likely to provide the required knowledge, skills and abilities, typically:
-
Bachelor’s degree in social work, mental health or a related field
-
1-2 years of experience providing direct field-based case management services
-
1 year of experience working with People Who Use Drugs (PWUD), People Experiencing Homelessness (PEH) and/or who have chronic health issues (preferred)
-
Commitment to the mission and values of the non-profit sector, with a passion for social impact and community service
-
Must have valid CA Driver’s License, automobile, and insurance
-
Must be able to successfully pass Live Scan (background check)

Required TB Test clearance
