This role is located in the D.C., Maryland and Virginia area. This role is ideal for dynamic professionals passionate about advancing senior care through innovative and client centered management practices. Working across the Aging & Amazing entity, you will enjoy flexible scheduling with part-time hours that have the potential to expand to full-time as the case load increases, involving in-person visits to client, support groups. Reliable transportation is required.
Key Responsibilities:
1. Comprehensive Care Management: - Conduct thorough in-home assessments and develop detailed care plans addressing clients' physical, emotional, social, leisure, and spiritual needs. - Monitor client health, coordinate medical appointments, and accompany clients to doctor visits. - Prepare and recommend health and wellness assessments, actively planning and coordinating the care plan implementation for each client.
2. Family Support and Education: - Provide expert guidance and coaching to family caregivers, improving their caregiving capabilities. - Maintain effective communication with family members and responsible parties, organizing weekly schedules of planned and completed activities. - Document all client interactions and care activities meticulously.
3. Marketing and Community Engagement: - Actively engage in marketing initiatives to attract referrals and inquiries, enhancing care management services. - Participate in community outreach and educational events to promote services and foster community support.
4. Professional Development and Networking: - Engage in continuous training and professional development. - Attend networking and educational events to support personal and professional growth. - Facilitate and participate in support groups and psychosocial workshops, providing valuable community services. 6. Operational and Team Collaboration: - Work collaboratively within a team setting, attending care management meetings and contributing to team goals. - Ensure all pertinent client information is accurately recorded and readily accessible.
Required Qualifications:
- Licensed Social Worker
- A minimum of 2 years of experience in Community Case Management, Hospice, or related field with additional experience in nursing homes is a plus.
- Must have reliable transportation for travel to and from client homes
- Background checks will be conducted upon hire.

- Strong communication skills (both verbal and written).
- Independent worker, self-starter, organized, detail-oriented with consistent follow through.

Pay: $25.00 - $40.00 per hour
