Job Summary:
We are seeking a skilled Dietitian to join our team and provide expert nutritional guidance to our patients. The ideal candidate will be educated in the two disciplines of Nutrition and diabetes education which possesses evidence of certification according to the requirements of the Board of Certified Diabetes Educators, and will have a strong background in patient assessment, diagnostic evaluation, and health coaching.
Responsibilities:
- Provides nutritional advice for various chronic illnesses including but limited to Diabetes, Obesity, Hypertension, Hyperlipidemia and Fatty Liver.
- Provides assessment and treatment plan for patients with diabetes on an individual basis.
- Provides each patient with educational material at the patient’s level of understanding according to their needs.
- Provides education according to the patients status in diabetes, i.e. new onset, gestational diabetic, patients with extended history of either type 1 or type 2 DM
- Provides case management for each diabetic patient, which includes keeping accurate records, patient follow-up at three-month intervals; monitoring diabetic control; ensuring timely medical visits, compliance with treatment and regime; follow up phone calls
- Provides patients instruction on how to use the glucometers.
- Instructs patients on insulin administration, timing and scheduling of medications.
- Provides patients with phone number for questions on their regime.
- Completes all necessary documentation.
- Participates in continuing education to remain current on medications and treatment modalities of diabetes care
- Collaborates with providers regarding patient’s diabetic management.
- Provides all providers with accurate information on all patients and collaborates with them
- Participates in Community Outreach and Health Promotion activities
- Schedules group sessions if needed.
- Other job related duties as assigned by supervisor
Requirements:
- Must be bilingual – English and Spanish
- 100% in office
- Bachelor of Science in Nutrition and Dietetics
- Current Illinois Professional License
- Registered with the Commission on Dietetic Registration
- Certified Diabetes Care and Education Specialist (CDCES) certification
- 3-5 years of experience in disease management and diabetes care
