Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need. Your role will involve assessing client needs, developing care plans, coordinating services, and advocating for clients. Responsibilities:
- Conduct comprehensive assessments of client needs and develop individualized care plans
- Coordinate and monitor services to ensure clients receive appropriate support
- Provide crisis intervention and counseling to clients in need
- Assist clients in accessing community resources and services
- Advocate for clients' rights and best interests
- Collaborate with other professionals, such as social workers and healthcare providers, to ensure holistic care
- Maintain accurate and up-to-date documentation of client interactions and progress
- Provide mentoring and guidance to clients to promote personal growth and self-sufficiency
Experience:
- Bachelor's degree in social work or a related field
- Proven experience in addiction counseling, crisis intervention, child welfare, or related fields
- Knowledge of motivational interviewing techniques
- Strong crisis management skills
- Ability to supervise and provide guidance to individuals or groups
- Familiarity with Medicare regulations is a plus
