The Service Coordinator will lead the development of service plans dedicated to providing all participants with all the services needed. SC will provide case management services and must be able to maintain documentation for each participant according to the policies and procedures established by the agency.
You'll be required to lead and attend several team meetings regarding the waiver participants and their care. Ideal candidate must have strong advocacy skills, ability to multi task and handle large amounts of paperwork. Must also effectively communicate both verbally and in writing. Must have the ability to make decisions and problem solve. Travel required.
Ideal candidates must have have experience with assisting with enrollment in Medicaid/Medicare or other services such as benefits, housing, healthcare, setting up appointments
Minimum qualifications:
Education:Degree in Human Services, Social Work, or related field/experience.
Experience: Minimum 1-year experience providing TBI/NHTD Service Coordination to individuals in New York.
Additional Requirements:
Possess excellent verbal and written communication skills.
Proficiency and experience with PC’s and Microsoft applications.
