Overview of the Role:
Privia is looking for a forward-thinking, organized, energetic Nurse Care Manager who delivers on objectives and seeks an opportunity to develop, implement, and deliver change within our medical group and affiliated partners. Working closely with the population health teams, networked facility partners, primary care providers, and practice operations at Privia Health, the Nurse Care Manager will provide extensive care coordination for Privia’s highest risk patients.
Primary Job Duties:
- Manage a caseload of patients who have been identified for care management
- Develop trust and build rapport with a select Privia patient population
- Establish a “call us first” relationship with these patients- patients are encouraged to call their PCP after hours and utilize preferred urgent care centers
- Engage patients telephonically, in office, in facility, or where needed
- Act as the liaison between the PCP and the patient
- Assist patients in accessing care at the appropriate source
- Have attention to detail for patients: medical, social, family, economic, and behavioral issues-work to mitigate these barriers
- Develop relationships with the care coordination teams, physicians, and clinic staff
- Lead case rounds, huddles, and PCMH Care Coordination for your patient panel
- Understand and utilize NextGen, population health tools
- Review patient non-compliance issues and conduct outreach to close them
- Maintain a courteous and helpful manner when working with patients, physicians, and associates
- Schedule your paneled patients for diagnostic testing and follow-up to ensure completion of testing
- Work in collaboration with Population Health Specialist
- Participate in special projects and perform other duties as assigned
Minimum Qualifications:
- BS in Nursing (or equivalent) and at least 2 years of care management or care coordination experience.
- Active unrestricted license in TX or able to obtain

TRAVEL is less than 20% from office to office.
