Housing Intake Coordinator

AFC has touched the lives of more than 5,500 youth through its programs, giving them hope, encouragement, and better equipping them for success in life. AFC’s mission is to inspire excellence and promote leadership to youth through mentoring, music and the performing arts in order to provoke positive change and enrichment in our community.
The Housing Intake Coordinator will be responsible for responding to referrals of new victims in
a timely manner in Houston, Texas’ Westwood Community (and surrounding areas; including
completing the intake eligibility form.
PRINCIPAL RESPONSIBILITIES:
- Serve as the initial point of contact for potential clients, providing information about program services and eligibility requirements.
- Conduct intake interviews with applicants to gather personal,housing, medical and other relevant information.
- Maintain accurate and up-to-date client files, including intake forms and assessment results.
- Collaborate with case managers and other staff to ensure a smooth transition for clients entering the program.
- Maintain a waitlist and prioritize applicants based on established policies and procedures.
- Provide referrals to external agencies or services as needed.
- Ensure compliance with data entry, documentation, and reporting requirements.
- Conduct thorough client orientations to review program rules, policies, expectations, and available support services.
- Guide clients through the lease agreement, explaining terms and conditions, and facilitate lease signing.
- Performs other duties/responsibilities or special projects as necessary by supervisor
QUALIFICATIONS:
- Strong interpersonal, communication, and active listening skills.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Knowledge of housing programs, community resources, and related services.
- Proficient in database management and MS Office applications.
- Strong organizational and time management skills.
- Sensitivity to cultural diversity and ability to work with diverse populations.
- Public speaking/training skills to effectively deliver client orientations.
- Excellent written, documentation and verbal communication skills
- Ability to work independently with a strong sense of focus
- Must have a valid Texas Driver’s License
- Must have reliable transportation
- Ability to legally operate a motor vehicle
- On-call availability required.
EDUCATION: Associates degree in Social Work, Counseling, Psychology or a related field
preferred.
HOURS: Some evenings and weekends required. 10% Travel
EXPERIENCE: Minimum of two (2) years experience performing client intake applications
and basic needs assessments with youth and young adults preferred.
SALARY: Starting at $35,000
