Hope Christian Health Center is a non-profit, faith-based organization operating two clinics in North Las Vegas, a mobile outreach team, and the HCHC Recuperative Care Center (HCHC RCC) in Las Vegas.
The HCHC RCC is a 38 bed inpatient program that offers
primary care, case management, wound care, and other supportive services to clients who are homeless made possible through partnership with the City of Las Vegas.

JOB SUMMARY: The Medical Records Coordinator is responsible for managing the medical records and EMR of the facility, including preparing, storing, and retrieving patient health records. The Medical Record Coordinator reviews medical records for compliance with approved policies. Updates the Medical Records System (EMR) as necessary) by thoroughly checking all data entry and notes.
DUTIES AND RESPONSIBILITIES:
Administrative Duties
- Prepares new patient charts by gathering necessary documents.
- Ensures medical records are accurately assembled and complete.
- Scans and stores paperwork in electronic medical records.
- Files paperwork and reports in patients charts promptly and accurately.
- Retrieves reports sent via email, transcribes them into the EMR, and uploads them for provider review.
- Manages incoming documents from multiple sources that include patient records, employment verifications, SNHD notices, insurance notices, FMLA/Disability forms Medical Records Requests etc.
- Reconciles the ECW Fax interface inbox: attaching documents to patient charts.
- Directs/uploads incoming faxes from Fax inbox into EMT ensuring documents are directed to the correct personnel or folders.
- Review all Release of Health Information Requests to ensure they are HIPAA compliant.
- Determine what information is being requested and gather the requested records.
- Initiate the process of requesting invoicing from accounting to requesting entity when there is an applicable charge.
- Once payment has been received the requested information is faxed to the requesting party ensuring accuracy and completeness.
- Performs clerical tasks such as answering phones, faxing, and assist patients walking in with inquiries pertaining to Medical Records.
- Ensures adherence to front office and clinic policies and procedures.
- Enters results into the EMR for provider review.
- Performs data entry for Colonoscopies, Cologuard, FIT Test results, Papanicolaou test, Mammograms, Outside Retinal Results, and outside labs.
- Properly attaches Diagnostic Imaging results to corresponding orders in the EMR.
- Maintains a safe and healthy work environment by following standards and legal regulations.
- Safeguards patient records to ensure compliance with HIPAA standards.
- Tracking medical records requests and following up with second and third requests, as necessary.
- Participants in educational opportunities to update job knowledge.
- Attends and actively contributes to staff meetings.
- Assists the outgoing medical records team member with tasks as needed.
- Performs any other duties as assigned.
QUALIFICATIONS:
Education:
- High school diploma or equivalent- required
- Associate's degree or higher in Health Information Management or Health Information Technology-preferred.
Experience:
- At least one year Medical Records experience-preferred
- Data entry experience-preferred
- Experience using eClinicalWorks, Azara, and Microsoft Office-preferred
- Healthcare system reporting experience-preferred
- Working knowledge of HIPAA-preferred

TYPICAL PHYSICAL DEMANDS:
Occasional standing, walking, lifting, reaching, kneeling, bending, stooping, pushing, pulling and lifting and/or moving up to 25 pounds. Frequent sitting. Manual dexterity. Good reading eyesight and vision for close work. Ability to communicate by speaking and hearing in person or on the phone. Ability to travel to various locations.
