Job Summary:
A Registered Nurse provides healthcare services to individuals living in a host home, group home setting, and independent living.
The job of a Registered Nurse centers on assessing individuals' health needs, creating care plans, administering/delegating medications and treatments, and monitoring individuals' progress. The RN will work closely with other participants of a person’s team including the person/guardian to ensure that the individual receives the appropriate medical care and support. The registered nurse will also provide guidance and education to families and staff to help them understand the individual's condition and how to manage their care.

Responsibilities:
A Registered Nurse has a range of responsibilities, including providing healthcare services and support to individuals with developmental disabilities or mental health conditions that are under the care of Wellness Healthcare Solutions.
The specific duties of a Registered Nurse may vary, but typically include:
· Assessing individuals' health needs and developing individualized care plans initially and at least annually thereafter unless a medical change indicates sooner.
o Comprehensive Nursing Full Body Assessment: A review of the individual’s body and a conversation about the individual's health (history and current) with the Guardian, LAR, staff, or individual
o SAMS Assessment: A review of the individual's capacity for self-medication. This evaluation will assist in deciding whether drug administration must be assigned.
o AIMS Assessment: A review of any side effects brought on by psychiatric medications, such as tremors, shaking, or any other form of uncontrolled movements (only for individuals who are taken psychotropic medications). (if applicable)
· Administering/delegating medications and treatments as prescribed by a physician.
· Monitoring individuals' progress and adjusting care plans as needed.
· Providing education and support to the individuals and staff to manage their conditions.
· Collaborating with other healthcare professionals, such as physicians, therapists, and social workers, and family members to ensure that the individual receive comprehensive care.
· Travel within the service areas to visit the individuals in there setting.
· Telephone Triage if applicable
· Focus Assessments as needed.
· Staff/Family Training if needed to ensure continuity of care.
· Supervision of LVN if applicable
· Possible afterhours calls in event of emergency.
· Completion of Service Delivery Summary (Nursing Notes)
o Including time. Location, credentials, individual’s name, and supporting documentation.
o For billing and payment purposes as well.
· Depending on the demands of the individual, more tasks can be necessary.
In addition, Registered Nurse may be responsible for maintaining medical records, ordering medical supplies, and communicating with individuals' families or guardians. They must also adhere to ethical and legal standards of nursing practice and maintain a high level of professionalism and compassion in their interactions with individuals and their families.
