Job Summary:
We are seeking a highly organized and detail-oriented Women's Coordinator to join our team. The Women's Coordinator will be responsible for providing care coordination and act as a liaison between the HOME program, OASIS, and the Lead Grant Evaluator for pregnant and postpartum mothers and their minor children from outreach through successful graduation. The Women's Coordinator ensures coordination of SUD, mental health, and physical health services. Monitors, tracks, and reports progress on grant goals and objectives. Coordinates with internal and external partners to support smooth transitions between waitlist, outpatient, emergency housing, residential treatment, and transitional housing.
Essential Functions:
· Supports the coordination of care for PPW and their minor children in all stages of engagement
· Participates in the project oversite committee for PPW grant
· Participates in the OnTrack Individual Care Team (ICT) at the request of the mother
· Collect data related to service delivery and family needs and outcomes
· Ensure grant participants receive the full array of recovery services
· Works collaboratively with families, community partners, and program staff
· Prepares, records, and manages individual and aggregate records and materials
· Maintains administrative records and provides administrative support
· Participates in Oversite committee and evaluates program strengths and weaknesses, progress toward goals, and opportunities for improvement
· Partners with Access manager, clinical housing manager, residential program manager, and OASIS medical team to ensure smooth transitions from initial outreach and early engagement throughout the treatment process
· Works closely with lead evaluator to ensure timely and accurate documentation and reporting
· Communicates with advocates, peer support specialist and ICT to ensure on-going engagement with services and assist in problem solving barriers to successful, sustained recovery
· Remains current with best practices to maintain a high quality of care
· Ensure ROI’s are in place for all resources and referrals as indicated
· Maintain professional ethics and boundaries
· Maintain patient privacy
· Serve as a mandatory abuse reporter
JOB REQUIREMENTS
Experience and Skills Requirements:
· Minimum of 2 years working with individuals in behavioral health settings
· Philosophical alignment with person-centered treatment and OnTrack’s mission, vision, and values
· Excellent organizational skills and time management skills
· Understanding of holistic dimensions of recovery and wellness, the impact of ACES, and Social Determinants of Health
Education/Licensure/Certification requirement:
· Bachelor’s degree in behavioral health preferred
· CADC or QMHA required or ability to attain within 3 months of hire
· Valid Oregon Drivers license with clean driving record
Background: Clear background check, including Medicare fraud check. Must pass drug screening test
Work environment: This job is primarily sedentary work and located in a standard office environment. Visual acuity is required for review of documentation, spreadsheets, and viewing of computer monitor. Occasional local travel to various job sites is necessary. Extended periods of work at desk, with extensive use of electronic communications
