Job Summary:
Maternal Infant Health Program (MIHP) is a home visiting program for pregnant women and infants with Medicaid Insurance. The program provides support services to women and caregivers so they have healthy pregnancies, good birth outcomes and healthy infants.
Job Description: Contract Position Registered Nurse (RN) and/or Licensed Social Worker
Responsible for providing care coordination in the client’s home that promotes healthy pregnancy, healthy birth outcomes, and healthy infant development during the first year of life.
Responsibilities/Duties:
- Complete a minimum of 2-5 home visits per week with pregnant women and/or infants who receive Medicaid Insurance and/or maintain a min. 3 client caseload.
- Provide comprehensive assessments for pregnant women and infants across multiple domains including but not limited to maternal and infant health risk, drug and alcohol use, domestic violence, depression and stress and safety and development.
- Provide case management, care coordination, support services, and education to clients and families using motivational interviewing techniques.
- Function as a member of a multi-disciplinary team of social workers, registered nurses and a program coordinator.
- Perform ASQ-3 and ASQ-SE questionnaires to track infant developmental milestones at appropriate months during the first year.
- Provide referrals and follow up for services, classes and specific needs of families
- Document all risks and information gathered at each home visit and submitting required documentation in MIHP EMR within appropriate timeframes.
- Perform all appropriate training modules required by the Maternal Infant Health Program (MIHP) and the State of Michigan prior to beginning clinical work.
- Attend via phone monthly case conferences
- Contact and respond to referrals of potential clients within required timeframes.
- Keep all client files updated/maintained in the MIHP EMR system within agency timeframes.
Required Education and Experience:
- Must hold a current Michigan license as a Registered Nurse or Social Worker
- 1 year maternal and/or pediatric nursing experience preferred

This is a contracted position and provider shall be responsible for all expenses incurred while performing services under contract. This includes license fees, memberships and dues, taxes, automobile and other travel expenses; meals and trainings, insurance, telephone and all salary expenses.
