Full Job Description

This position is in part a grant funded position and is contingent on the renewed funding at the end of each award period.

JOB SUMMARY:

The OB Program Manager is a full-time collaborative professional position for someone with outstanding people, organizational, and project management skills.

This OB Program Manager will enhance community outreach and engagement for the pregnant and postpartum population through various grant and non-grant funded initiatives including but not limited to three grant-funded program arms outlined as follows: 1) leading the planning and implementation of the Supportive Pregnancy Care group prenatal program through the March of Dimes, 2) educating providers about new internal referral workflows related to peripartum psychiatric services and working with the IT department to track referrals and patient engagement, and 3) planning and implementing prenatal and postpartum visit adherence incentives (ex/ pack n’ plays, diapers, etc) as laid out by the MCHRC Grant #2022-15. Furthermore, the OB Program Manager is responsible for working with the Chase Brexton business intelligence and grants teams to manage and meet the grant reporting requirements.

This position reports directly to the Director of Reproductive Health.

MAJOR DUTIES AND RESPONSIBILITIES:

Analytical and Critical Thinking

Represents CBHC in external discussions and partnerships for matters related or tied to CBHC grant-related programs as described above.
Gathers and analyzes social determinants of health (SDOH) and demographic data, generates reports, and determines gaps in data collection and reporting.
Participates in the development and implementation of the MCHRC Grant #2022-15 strategic and quality plans which align with CBHC strategies and objectives.
Participates in grant development, implementation, budgeting, billing, reporting, evaluation, quality improvement, and workgroups.
Oversees administrative functions for the arms of the MCHRC Grant #2022-15 (as described above) and implements processes for improved procedures and customer service, in conjunction with other staff and stakeholders.
Monitors reporting requirements for internal and external stakeholders, funders, community partners, and regulatory entities.

Communication

Serves as a central point of contact for the MCHRC Grant #2022-15 initiatives on day-to-day programmatic, operational, and administrative matters.
Helps to coordinate Chase Brexton working groups related to grant and non-grant funded service provision, ensuring staff, patients, community, and stakeholder voices are integrated throughout.
Requires outstanding communication skills with staff, patients, and community members.
Represents Chase Brexton at internal and external workgroups and community gatherings and shares information appropriately.
Liaison to MD Dept of Health and other county health departments as needed

Planning and Organizing

In conjunction with the grants team and/or marketing team, is responsible for reviewing and modifying grant narrative and deliverables as needed.
Works with marketing, operations, group prenatal care clinicians, and support staff to advertise and recruit for programs such as group prenatal care and visit adherence incentives.
Helps to coordinate and attend March of Dimes Supportive Pregnancy Care training session and works closely with March of Dimes to obtain all necessary supplies and patient education materials for group prenatal sessions
Creates and implements a group prenatal scheduling template with support of supervisor and operations and creates and implements workflows around patient recruitment, scheduling, and patient care coordination.
Coordinates logistics for prenatal groups, including reserving the meeting space, ordering and providing snacks for participants, developing a patient education curriculum with support materials, coordinating guest speakers, setting up and storing clinical tools (doppler, blood pressure machine, exam station), cleanup after the conclusion of the sessions.
Keeps detailed schedules and records for supply ordering, invoices, and expenditures related to group prenatal care and antenatal and postpartum visit adherence incentives.
Tracks adherence of all peripartum patients to prenatal and postpartum visits, develops and manages the visit adherence incentive program.
Works closely with Perinatal psychiatric provider and Director of Psychiatry to create and improve upon referral workflows, documentation and reporting related to perinatal psychiatric services.
Supports the success of the group prenatal care initiative, perinatal psychiatric services, and visit adherence incentives by working closely with clinicians, support staff, nursing and operations.
Works closely with nursing and social work to link pregnant and postpartum individuals to wraparound support services, eligibility and enrollment services for third-party payment, and community-based programs for pregnant women and caregivers.
Serve as an internal voice for the community by improving responsiveness and accessibility of the initiatives outlined in the grant.
Manage ad-hoc tasks as requested by supervisors or Senior Leadership Team members, which may include programs such as implementing mom/baby visits for the first year of life, as an example.
Continue creating newborn cases for UDS data management for OB indicators

Teamwork

Works collaboratively with supervisor, grants team, billing, Senior Leadership, and other internal departments to meet program and funding deliverables and metrics.
Works closely with marketing, EDI, and other internal and external stakeholders to monitor community input/feedback and modify care delivery accordingly.
Participates in interdisciplinary team meetings as needed, including Ob team meetings.
Prepared to serve as primary or back-up co-facilitator of group prenatal visits.

Compliance, Policy, and Procedure

In collaboration with other staff, reviews policies and guidelines to ensure standards of care are consistently delivered within the group prenatal care setting and meet the needs of community members.
Monitors, evaluates, and updates data needed regarding grant reporting to ensure continuous quality improvement.
Maintains awareness of Federal and State regulations, funding, organization policies and procedures, and internal policies and procedures to ensure legally compliant delivery of service.
Serves as primary point of contact, participates in, organizes, and is responsible for external and internal audits and site visits.

Checking, Examining, and Recording

Keep budgets and grant tracking documents up to date.
Oversees all expenditures, invoices, and expenditures related to group prenatal care and antenatal and postpartum visit adherence incentives.
Keeps accurate departmental statistics and reports on services.
As applicable, documentation in client medical records aligns with organizational expectations and requirements.
Works closely with the billing department to ensure that group prenatal visit billing is complete and accurate.

Computer proficiency and web-based skills in Outlook, Survey Monkey Microsoft Excel, Word, and Power Point (Electronic Medical Record, specifically Athena, preferred).
Excellent written and oral communication skills are needed.
Decisive and collaborative approach to decision-making

EDUCATION AND/OR EXPERIENCE:

Required: Registered Nurse license with AND or BSN, MSN preferred, and experience working in public health, health sciences, health administration or related field.
Minimum 2 years’ experience in OB nursing, community health, outreach and health education, group prenatal care, or a combination of above fields.
Demonstrated understanding of cultural values and norms of various communities, particularly of various racial and ethnic backgrounds including Latina communities, and sensitivity to and understanding of barriers and considerations which may contribute to lack of access.
Spanish proficiency preferred.
Individuals with personal history and experience relevant to priority communities and services provided are encouraged to apply.

WORKING CONDITIONS/PHYSICAL DEMANDS:

Work is typically performed in an office environment, primarily at MVC and GBC but services may extend to any CBHC site and may require engagement events in the community.
Must have a valid driver’s license and transportation.
Ability to commute between clinic sites, primarily MVC and GBC.
Be able to lift 30-50lbs for group prenatal care set-up and visit adherence incentives.
