JOB SUMMARY
The BTH Case Manager provides supervisory, on call services, client caseload responsiblities, life changing services to youth in care. They seek to provide permanency plans for each child on their assigned caseload by advocating for the children. These activities relate to supervisory, teamwork, group work, work with family according to the treatment plan, family and custodian contact, community resources, and case record management.
RESPONSIBILITIES
1. Placements:
- Review all received information on placement, including completion of an admission assessment and any other evaluation of a child for placement
- Obtain common application (2087 or 2087EX) and placement authorization (2085FC, K-908-2279 & attachment A) prior to/at time of placement from managing conservator
- Coordinate placement time/date with House Parent and Child Protective Services
- Complete and submit placement packet according to BTH Policy and Procedures
- Ensure all medical/dental/therapy needs are addressed
- Make 7-day contact with family and child(ren)
- Ensure 72 hours is completed and submitted (when instructed by DTS and PLSP)
- Ensure 30 day is completed and submitted (when instructed by DTS and PLSP)
- Ensure House Parents are trained on writing progress notes and the use of Extended Reach
- Will be responsible for on-call services as designated by the Program Administrator/LCCA
- Will have management and supervisory responsibilites over house parents that will include but not limited to performance evaluations, mangement of job-descriptions and/or day to day responsbilities as well as any other supervisory management duties assigned from the Program Administrator/LCCA
- Other duties as deemed necessary
2. Case Management:
- Complete all required training as outlined in BTH Policy and Procedures with new children assigned to caseload
- Monitor the House Parents and children on your caseload through daily or weekly face to face visits. You must visit with each child and House Parent individually on a daily or weekly basis and maintain documentation on the contact sheets. If the child is able to communicate in a meaningful way, the contact with the child must:
- Be for a length of time sufficient to address the child’s needs and determine the appropriateness of the placement
- Provide an opportunity to meet in private; and
- Provide an opportunity for the child to express his/her feelings about how the placement is working out
- If the child is non-verbal or pre-verbal, the contact with the child must be for a length of time sufficient for an appropriate observation of the child’s placement, including verification that the placement is meeting the child’s needs as specified in the treatment plan. The Program Administrator/ LCCA must review and approve documentation of contacts.
- Provide and document ongoing training to House Parents as required and needed. House Parents will be notified of all trainings available to them through the BTH training program
- Monitor BTH to ensure compliance with licensing standards and BTH Policies and Procedures (i.e.: inspection, etc.)
- Ensure BTH home and child notebooks are current and in order according to BTH Policy and Procedures
- Develop treatment plans based on information gathered through ongoing contact with family and child(ren). This is in compliance with the assigned level of care
- Schedule and conduct treatment plan staffings with House Parents and child(ren). Notifications are to be sent out to appropriate parties. Treatment plans must be approved and distributed as required
- Ensure House Parents are providing care according to the level of care
- Attend all PPT meetings or notify supervisor if unable to attend
- Attend al ARD meetings or notify supervisor if unable to attend
- Complete and submit action sheets according to BTH Policy and Procedures
- Complete monthly reports on BTH/child(ren) in accordance to BTH Policy and Procedures
- Maintain contact with Child Protective Services as needed (min. of one contact per month)
- Complete and submit Serious Incident Reports as required and needed
- Ensure Extended Reach is accurate and updated
- Monitoring and providing support services to House Parents, including the initiation of development plans, corrective actions, or adverse actions
- Approve any restrictions that will be imposed on a child for more than 7 days that have not been reviewed and approved by the treatment director or service planning team, and any monthly re-evaluations of restrictions that continue for more than 30 days.
- Approve any restrictions to communication and visitation with the child’s family that are imposed on a child, but have not been reviewed and approved by the treatment director or service planning team, including monthly re-evaluations of restrictions that continue for more than 30 days
- Approve any restrictions to a particular room or building for more than 24 hours that are imposed on a child, but have not been reviewed and approved by the treatment director or service planning team
- Other duties as deemed necessary
3. Discharges:
- Complete Action Sheets and submit according to BTH Policy and Procedures
- Complete Discharge Summary and submit according to BTH Policy and Procedures
4. Miscellaneous:
- Maintain the cell phone with on call status as required by the Program Administrator/LCCA
- Provide crises intervention as needed
- Ensure child’s level of care is current and appropriate documentation is submitted when attention is needed
- Attend related school functions/meetings as deemed appropriate and necessary by supervisor
- Complete and submit weekly time sheets to the Accounting Department
- Keep an accurate account of travel and submit a travel log to the Accounting Department by the 5th of each month
- Complete required hours of annual training per year as determined by policy
- Maintain a professional working relationship with all people at all times
- Keep Program Administrator/LCCA current on status of caseload
- Other duties as deemed necessary
5. Case Managers Shall Participate in:
- Staffings (Permanency Planning Team (PPT)) required by FPS
- Family Group Decision Making Conference Meetings required by FPS or a court
- Any other meetings required by FPS or a court
KNOWLEDGE, SKILLS, AND ABILITIES
- Must have thorough knowledge of the Bridge to Hope policies and procedures, DFPS Minimum Standards, Contracting, as well as all other state and federal regulations
- Demonstrate understanding, implementation, and ability to maintain current knowledge of the treatment plan and special needs of each youth and to ensure documentation of progress of the youth for their treatment plans
- Ability to manage time, prioritize, and respond to the needs of children effectively in an organized and structured routine
- Ability to work cooperatively and productively with youth and co-workers to effectively resolve conflicts appropriately and use the chain of command when communicating
- Ability to provide constructive and developmental feedback in a professional manner that demonstrates the concept for change and gives examples and suggestions to those receiving the feedback
- Ability to grow professionally and learn new skills; to accept constructive feedback and modify behavior accordingly
- Ability to lead others in working in a stressful and sometimes unpredictable environment; skilled in handling crisis situations without overreaction or loss of objectivity
- Possess sensitivity to the cultural differences that are present among the youth and co-workers
- Be physically able to perform the approved Behavior Management System
- Meet all of the employment qualifications required
QUALIFICATIONS
- Must have a minimum of a Bachelor’s degree from an accredited college or university
- Must be at least 21 years of age
- Ability to read and comprehend simple instructions, short correspondence, and memos as well as the ability to communicate effectively both verbally and in writing
- Ability to deal with standardized situations with occasional variables
- Must possess a valid driver’s license and have a good driving records which allows insurance coverage by BTH’s auto insurance policy
- Must submit to and pass a pre-employment drug test as well as random drug tests during employment
- Must obtain a recent tuberculosis test verifying no active tuberculosis
- Must authorize BTH to check employment references and to conduct a background check in accordance with requirements by the state
- Within two years from date of hire, this position will require candidate to obtain the Child Care Administrator License (LCCA)
