Looking for a Nurse Practitioner, Physician's Assistant, MD, or DO

Job Description: Part-Time Onsite Medical Provider

Open Position:

Our visionary team is seeking a Part-Time NP, PA, MD, or DO for our Disease Management Program. This is an Individual Contractor (1099) position. The ideal candidate will be willing to work independently and with the Director of Disease Management, Chief Medical Officer, and Director of Operations to provide outstanding employee/patient care. Candidate will be patient/client centered, flexible and gradually take ownership as a company nurse practitioner. We are a company with an entrepreneurial spirit and a passion for restoring the patient-provider relationship.

Duties and Responsibilities:

Providing and managing disease management care services, including assessing, diagnosing, prescribing, treating, and educating patients.
Works closely with physicians and other clinicians in a team approach to patient care.
Assesses patient status by obtaining health history through patient interviews and chart reviews; assessing illness, risk factors, family history, psychosocial situation, and cultural factors; and performing appropriate physical examination.
Orders appropriate laboratory diagnostic and other screening tests. Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness.
Integrates data to determine diagnoses and therapeutic plans utilizing evidence-based medicine algorithms for treatment of care, including identification of any health risks.
Develops and implements treatment plans by prescribing medications in compliance with medical practice guidelines and state laws.
Instructs patients regarding medications and treatments. Educates patients regarding health promotion and disease prevention.
Communicates appropriate case management information to other professionals and nursing staff for coordination of care.
Prepares documentation for medical records including updating patient medical chart by posting examination and test results, diagnosis, medications, and treatment in computerized manner.
Participates in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures.
Acts as consultant to clinical team on area of specialty. Complies with patient confidentiality requirements. Promotes patient advocacy.
Collaborates with multidisciplinary team members by making appropriate referrals.
Performs other duties as assigned.

Performance Requirements:
Knowledge:

Knowledge of advanced practice of nursing including medical terminology, anatomy and physiology, pathophysiology, pharmacology and therapeutics, basic nutrition, behavioral factors, psychosocial/family systems, diagnostic testing, interpretation of results and clinical decision making, alternative treatment modalities.
Knowledge of professional nursing theory, practices and regulations related to evaluating and providing patient care, and professional ethics related to the delivery of nursing care.
Knowledge of medical equipment and instruments used to administer patient care.
Knowledge of common safety hazards and precautions to maintain a safe work environment.
Knowledge of health promotion, health risk identification, and patient education principles and techniques.
Knowledge of community, professional, and educational resources.
Knowledge of practice management systems including electronic health record systems (EHRs).

Skills:

Skills in oral and written communication; interviewing to gather medical histories; providing presentations, consultations, facilitation, and documentation, e.g., writing medical orders, patient education materials, and medical record updates.
Skills in case management, time management, problem solving, crisis intervention, multi-tasking, and other organizational matters.

Abilities:

Ability to analyze complex data and draw conclusions.
Ability to interpret, adapt, and apply guidelines and procedures.
Ability to establish and maintain effective working relationships with patients, clinical staff, and the public.
Ability to competently use Microsoft Office, including Teams, Word, PowerPoint, Excel, and appropriate practice management software.

Equipment Operated:

Standard medical examination room equipment and instruments. Using a laptop/health information management system.

Work Environment:

Exam room with frequent exposure to communicable diseases and other conditions common to a clinic environment.
