Full Job Description

EMPLOYMENT STATUS: Part Time or Full time for counselors

LOCATION: NYC Brooklyn

Overview: Community Integration Counseling (CIC) is an individualized counseling service designed to assist the waiver participant to effectively manage the emotional difficulties associated with adjusting to life after a traumatic brain injury or another adjustment related illness/disability. It is a counseling service provided to a participant coping with the need to revise long term expectations, his/her changing roles, and the impact of these changes on him/her, family members and informal supports. This service is provided in the provider’s office or the participant’s home. It is available to participants and/or anyone involved in an ongoing significant relationship with the consent of the participant when the issues discussed relate directly to the participant. The participant must be present (face to face) at the time of service delivery. “Collateral counseling” is not permitted without the participant present. Regarding client confidentiality, the sharing of information obtained during a CIC session can only be disclosed in accordance with accepted professional standards regarding client confidentiality.

QUALIFICATIONS: Persons employed as a Community Integration Counselor must be a/an:

1. Licensed Psychiatrist (licensed by the NYS Education Department);
2. Licensed Psychologist (licensed by the NYS Education Department);
3. Licensed Master Social Worker (LMSW- licensed by the NYS Education Department);
4. Licensed Clinical Social Worker (LCSW- licensed by the NYS Education Department);
5. Individual with a Doctorate degree in Psychology;
6. Licensed Mental Health Practitioner (Licensed by NYS Education Department);
7. Certified Rehabilitation Counselor
(Certified as a Certified Rehabilitation Counselor (CRC) by the Commission on
Rehabilitation Counselor Certification);
AND
Must have, at a minimum, two (2) years of experience providing adjustment related counseling to
individuals with Traumatic Brain Injuries, disabilities and injuries

Acceptable verification of credentials related to qualifications. Applicants for this
position acknowledge their ability and willingness to carry out all duties outlined in this job description.

DUTIES AND RESPONSIBILITIES:

Provide education and counseling to individuals with a variety of disabilities and their natural supports as specified in their approved service plan.
Implement Detailed Goal Plans derived from the assessment by meeting with each participant as indicated in their service plan and monitoring progress on goals.
Complete session notes for all encounters with the participants, required documentation after each counseling session and submit to supervisor on a weekly basis.
Complete progress notes as needed for contact with Service Coordinator and other service providers.
Complete individualized assessments, as necessary.
Assist participants in developing coping strategies to adjust to life challenges.
Complete Individual Service Reports (ISR) as required to document goals, progress, and recommendations.
Attend and participate in scheduled Team Meetings for participants served to review the current Service Plan twice annually as scheduled by the Service Coordinator. The CIC is expected to notify their supervisor in the event they are unable to attend a scheduled meeting so that coverage by the supervisor can be arranged.
Interface with the Service Coordinator and all other service providers to maintain communication regarding the progress of each participant served.
Meet with supervisor at least bi-weekly for regular supervision and feedback.
Mediate emergency situations pertaining to the participant according to established policies and procedures and ensure communication of such situations in a timely fashion.
Ensure that each participant is treated with dignity and respect in accordance with New York State regulations and Agency policy.
Report possible violations of participants’ rights promptly, and complete and submit incident reports in accordance with New York State regulations and Agency policy.
Assist with any investigation into incidents occurring within the program as requested and ensure that confidentiality is maintained in regard to such investigations.
Maintain the confidentiality of any information relating to the program participants and staff of the program.
Ensure that the participant’s needs are met and intercede on their behalf as necessary.
Attend and complete training in accordance with New York State regulations and Agency policy.
Perform additional responsibilities as requested and/or necessary, including staffing alternate locations when assigned.
Providing necessary services as required for the participant.
Assess the participants and evaluate progress with supervisor.
Collaborate with all disciplines to plan and evaluate team goals for each participant.

Job Type: Part-time

Pay: $35.00 - $40.00 per hour
